Headquartered in Madison, TN, the company (resulting from the combination of SunCrest and Omni Home Health) offers a comprehensive and coordinated array of home health services to its patients including skilled nursing, physical therapy, home health aide, speech therapy, occupational therapy and medical social work, as well as private duty nursing care and outpatient rehabilitation therapy. The Company serves a census of approximately 8,000 patients and operates in 76 branch locations across 11 states including Florida, Tennessee, Georgia, Pennsylvania, Mississippi, Alabama, Missouri, Ohio, Kentucky, Indiana, and Illinois. Furthermore, the Company operates in attractive Certificate of Need (“CON”) states. In addition to home health branches, SunCrest operates four private duty branch offices in Tennessee, as well as four outpatient therapy clinics in Florida. In 2013, MBF sold its investment in Suncrest to Almost Family, Inc., one of the largest home health providers in the United States.
